The downstairs is concrete floor with tile on top an then floated for this glue down LVT

It's even worse upstairs with the regular wood subfloor

I had a company install LVT over a floated tile floor now I'm seeing gaps everywhere on the slab floor! As well as my upstairs where they didn't have to float! What the heck is going on?! Please help
