Hot air vent behind the dry wall

Dear All,

I am in the process of ripping off my carpet to install new hardwood flooring on my top floor. After removing the baseboards in one of the rooms, I saw an air vent positioned behind the dry wall (please see picture). Was quite surprised to see and was wondering if this is done for a reason ? I checked and saw that hot air does indeed come through the vent, but in the dry wall !! Was this done wrong and how can i correct this ?

Probably had a heat diffuser and someone just didn't put it back after the carpet was installed because the carpet installer forgot to cut out the hole.
Something like this one.
