I just finished a 900 sf recoat and after the first coat 4 instances of this occurred-

Top 1/16" hardwood ply...there were some other apparent delam problems that I repaired prior to recoating, these caught me by surprise.

How did you repair the delam parts? Was it on the bond line or not? Could you see the adhesive?

Cut it off and glued it together, yes, none that was apparent.
