Boy, that's ugly. I'd rip the flooring out and reinstall with all new substrate.

Ever seen a carpet patch installed with one inch long, 1/4 crown, 18 gauge staples? The renters dog dug into the carpet........ the pad too, so before stapling down the carpet patch, the guy replaced the missing pad with 1/4 inch plywood.
........ in a hall doorway no less. Not hard at all to make that look better.

I've seen patches put in with shingle nails, does that count? Matter of fact my own mother did that. It worked until I came to visit the next summer.

Seen nails too, just first time seeing missing pad replaced with plywood.
Yep, they usually use an upside down carpet scrap.
Plywood and upside down carpet scraps look the same when you're doing crack.
